Our Medical services include but are not limited to: 

  • Preventive Health
    • Adult
    • Children (including vaccines for private insurances; we do not participate in Vaccines For Children, a Medicaid program)
  • Chronic disease, including these and more:
    • Diabetes
    • Hypertension
    • Thyroid Disorders
    • Digestive Health
    • Chronic Pain (in conjunction with pain consultants)
    • INR/Coumadin management and checks
    • Hypogonadism
    • Heart Failure
    • Allergies/hay fever/postnasal drip
    • Arthritis and autoimmune arthralgias such as psoriatic arthritis and rheumatoid arthritis
  • Women's Health (in conjunction with Gynecologists when appropriate)

  • Lab Collection (for our patients only)

Bonifay Fees:

  • Insurances: rates, copays, and deductibles are dependent on your specific plan
  • DOT Physicals: $100 credit or cash (or check if a regular patient)
  • Self-pay office visits (initial or followup): $90, cash or credit
  • Sick child (under 18) visits are $50 if not using insurance
  • Simple school/sport physicals: $25 credit or cash (or check if a regular patient)
  • More complex physicals (requiring EKGs, labs, etc., as in for pre-employment) are $50 credit or cash (or check if a regular patient). Ancillary services are charged per additional item (urine tests, EKGs, blood tests, etc.)
  • Procedures can be done for self-pay patients, please inquire about pricing in the office
  • For our self-pay patients, we are keen on using the discount drug lists available at various pharmacies. We also are able to collect labs and have them performed at a discount rate for our self-pay patients. There are several radiology centers in the region that have greatly discounted cash rates; we are happy to point you in their direction.
  • We only take checks from established patients

Insurances:

Insurances that we can accept vary by provider, but we are able to accommodate the below insurances in the office:
  • Medicare
  • Florida Medicaid; this does not include the individual HMO plans
  • Blue Cross plans
  • Aetna 
  • Humana
  • United Healthcare
  • Tricare
  • Cigna
  • ChampVA
There are other, less common plans that we see; just ask!
